Personal Profile
This is the first screen you enter when you run MediBase. Enter your personal profile here. Make sure the data you enter is accurate. Use the buttons at the top to navigate around the program. Data is saved on the fly, so you won't lose any information.

Physician
Your physician and pharmacy information goes here. Information entered here and in the Personal Profile screen will display in the Patient Summary.

Medical History
This screen stores your medical history. Check the boxes that apply to you. If you are unsure about anything, contact your physician to see if they have the information. Enter any other pertinent information in the "Other" box. This information will show in the Report.

Medication
Keeping track of your medication is important. Using this handy database, you can enter your medications, dosages and expiration date to keep permanent records of them. If you want MediBase to remind you to take your medicine, click the "Reminders" button at the bottom of the screen. You can set up to 4 reminders, with the option to email you reminders if you choose. The program must be running for this feature to work.
MediBase will also monitor the expiration dates of your medications. If it detects a medication has expired, it will flag it "EXPIRED" and pop up a warning dialog. Once you have replaced the medicine, you can edit the entry, and change it to the new expiration date.

Donor
Keeps track of your donor status. You can choose not to be a donor, donate all useful organs, or choose which organs you want to donate.

Donor Cards
To print out donor cards that you can cut out and carry with you, click "Print Donor Cards". You and 2 witnesses can then sign them to legally let others know what your wishes are in the event of your death. If possible, print them on heavy stock. Then cut them out and keep in pocket, wallet or purse.

Report
MediBase will print out a full report that you can take to the doctor with you, or file away with your other important papers.
